Submissions must meet all of the following requirements:

• Description may not include any Protected Health Information (i.e. information that cannot be lawfully disclosed without patient authorization).
• Details included in the submission must be in compliance with clinician’s company or establishment policies.
• On-line submissions must also include valid e-mail address.
• Description must be original, unpublished, the sole property of submitter.

General Conditions: By submitting a success story, the submitter accepts and agrees to these requirements. By submitting a success story, the submitter agrees to allow use of his or her name and/or photograph for advertising purposes (including posting of first name last initial on the web) without compensation unless prohibited. Nestlé may contact submitter to further understand success story if submitter’s entry is selected to be featured online as a “Clinician Success Story” or “Clinician Spotlight.” All stories and all rights of ownership (including the right to edit, modify, adapt) and publication of same will become the property of Nestlé and will not be returned.

Nestlé Nutrition assumes no responsibility for computer system, phone line, hardware, software or program malfunctions or other errors, failures of any kind, delayed computer transmissions or network connections that are human or technical in nature. Nestlé Nutrition reserves the right in its sole discretion to void a success story submission, in its entirety or in part should virus, bug, tampering, unauthorized intervention, fraud, technical failures, or other causes beyond Nestle's control corrupt the administration, security, fairness or integrity of the submissions. Nestlé Nutrition is not responsible for incorrect or inaccurate submission information whether caused by Internet users or by any of the equipment or programming associated with or utilized in the site by technical or human errors, which may occur in the processing of the submissions.
